Cardioversion
Cardioversion
Date/Time: []
Indication: [SVT/unstable tachycardia/Afib]
Performed by: Self
Verbal consent was obtained due to emergent nature of presentation. The patient was placed on continuous cardiac monitor. AED pads were placed and connected. All necessary supplies for potential airway intervention and cardiac decompensation were immediately accessible in the room. If necessary, sedation was provided as documented. The AED was set to synchronize and charged. Once all staff were cleared from the patient/bed, a shock was delivered. The post-procedural rhythm was [sinus]. Hemodynamics were improved and blood pressure was stable immediately after the procedure and there were no complications.

Emergency consent was invoked due to the nature of presentation. The patient was placed on continuous cardiac monitor. AED pads were placed and connected. All necessary supplies for potential airway intervention and cardiac decompensation were immediately accessible in the room. The AED was set to defibrillation and charged. Once all staff were cleared from the patient/bed, a shock was delivered. The post-procedural rhythm was unchanged after the initial defibrillation. Subsequent attempts were made and the final rhythm after further defibrillation attempts was [sinus].
