A time out was completed, verifying correct patient, procedure, site, positioning, and equipment. Signed consent in the chart.
Patient’s [L/R] groin area was surveyed with ultrasound to identify appropriate landmarks. Chlorhexidine was applied topically to the skin. Using a 25 gauge needle (aspirating during insertion), 2mL bupivicaine was injected just under the skin as local anesthetic. Using a [20ga tuohy] needle under ultrasound guidance with constant aspiration while advancing, 15mL of 0.5% bupivacane was injected just under the fascia illiaca, lateral to the nerve. Distinct blackening was noted pushing down the nerve and coarsing up the fascia illiaca. The iliopsoas was pushed downward. The patient was counseled on signs/symptoms of LAST.

Using a [20ga tuohy] needle under ultrasound guidance with constant aspiration while advancing, [10]mL of 0.5% bupivacane was injected just under the fascia illiaca, lateral to the nerve. Distinct blackening was noted pushing down the nerve and coarsing superiorly and inferiorly along the fascia illiaca plane. The iliopsoas was pushed downward. Immediately after the procedure, decreased sensation was noted over the medial, anterior, and lateral thigh indicating that all three nerves(femoral, lateral femoral cutaneous, and obturator) were adequately anesthetized. The patient was counseled on signs/symptoms of LAST.
