Using anatomical landmarks of the occipital prominence and the mastoid process, the greater occipital nerve’s location was estimated. There was point tenderness over this area. The area was prepped with chlorhexidine and hair was manipulated out of the way. A 25ga needle was advanced at a 45 degree angle under constant aspiration until bone was encountered. The needle was then withdrawn 1mm and 1cc of 1% lidocaine was deposited. The needle was then withdrawn and redirected laterally and medically under constant aspiration. Each time, another 1cc of lidocaine was deposited. The needle was then withdrawn. Bleeding was controlled. The patient reported almost immediate improvement in symptoms.
